Radiology Vision and Target Operating Model

Update: The Target Operating Model (TOM) was endorsed with feedback by the Imaging Executive Board (IEB) on 15 April 2021, and was approved by the Board of Chief Executives on 1 June 2021.

Aim

To help achieve a sustainable and resilient radiology service, work to develop a shared vision and Target Operating Model (TOM) was approved by NHSScotland’s Chief Executives Group on 4 August 2020. The Imaging Executive Board (IEB) has asked the Scottish Radiology Transformation Programme (SRTP) to support this work.

The aim is to produce a blueprint for the future of radiology in Scotland, which is owned by the radiology community and informed by the experiences of patients, carers and referrers. 

While good progress has been made to address the significant demand and capacity issues facing the radiology service in Scotland, the situation remains precarious and the need for national collaboration and coordination has become increasingly urgent as a result of the COVID-19 pandemic. 


Objectives

To develop an agreed model for the future radiology service in Scotland to be ‘a world class, person-centred sustainable Radiology service that continually improves the health and wellbeing of the people of Scotland’.


How

  • Agree the Vision and Principles
    • Building on previous engagement, find out the wider radiology community and referrer views on the shared vision statement and principles, to ensure that they reflect a future radiology service that is realistic and will achieve our goal of becoming more sustainable and resilient.
  • Define the Current Operating Model (COM)
    • Understand what the baseline of current radiology services within Scotland is and provide a greater understanding when building the future Target Operating Model (TOM).
  • Agree the Target Operating Model (TOM)
    • Achieve a nationally agreed TOM, which aligns with the shared vision and will ensure sustainable service delivery that can handle disruption, including economic crisis such as COVID-19.
  • Detail the roadmap to achieve the Target Operating Model (TOM)
    • Developing a high level roadmap for radiology in Scotland that details the incremental and sequential moves to the agreed target state. 

Who

Delivery of the project includes widespread engagement with those experiencing, referring and operating within radiology services in Scotland.


Recent Activity

An agreed vision statement and set of principles

To engage with the wider radiology community, a survey containing a developed vision and a set of principles based on previous engagement was disseminated. 278 stakeholders completed and returned the survey, with over 90% agreeing that the vision statement and principles reflected a future radiology service in Scotland that was realistic and would achieve our goal of becoming more sustainable and resilient.

The agreed vision and principles was approved by the Imaging Executive Board (IEB) on 10 December 2020.

Defining the Current Operating Model (COM)

A Current Operating Model (COM) package was created focusing on:

  • A high level timeline of the patient journey within radiology,
  • A SWOT and PESTLE to analyse the radiology model from an external perspective,
  • The COM Canvas – a high level visual of what we think the overall system looks like describing the core layers of the current model.

The COM was validated by Service Managers and Clinical leads, before being validated and accepted by the IEB in February 2021.
